MONTHLY PRICE FOR CLASS MEMBERSHIP, PRORATED ON FIRST PAYMENT $80.00

 

I declare that I am the parent or legal guardian of the child listed above, and I have authority to sign agreements and make decisions on the child’s behalf.

 

 

I hereby agree that Simply Ballet, Simply Ballet LLC and all its locations, all theirs officers, contractors and employees or volunteers, are not responsible and will be hold harmless from and against any liability for any injury, contraction of infectious disease, death or loss of property that may occur to myself, my child or any person while taking class, competing, participating in recitals, special events, demonstrations or shows, for any reason whatsoever, including ordinary negligence on the part of Simply Ballet. In case of emergency, my child may receive medical transportation and be treated by a qualied physician. As the parent/legal guardian, I give my consent for my child to participate in programs at Simply Ballet. I understand that participation in dance and related activities may result in unavoidable injuries due to motion and unique activities of a dance class environment.

 

I agree that I will not bring my child to class or camp if the child is sick or is suspected of carrying an infectious disease or virus, including but not limited to Influenza, COVID-19, Hands Foot and Mouth Disease, Pink Eye, etc. Simply Ballet may ask me or my child health questions if my child exhibits any signs or symptoms of being unwell at any time during the program.

 

I acknowledge that there may be video and or sound recording or photographs of my child taken during their class and activities organized by Simply Ballet and I grant permission for the video, images and sound to be used for training purposes and promotional advertising for Simply Ballet, including social media, without compensation. I hereby waive all rights of publicity on behalf of my child in connection with the use of this media to promote Simply Ballet. I acknowledge that I may request a Simply Ballet employee that my child be excluded from any photographs or videos.

 

In submitting this form I agree to pay any applicable tuition or fees. In the case of class membership, I will be charged the first payment today, and I understand that I am agreeing to enroll in a monthly automatically renewing subscription and monthly payments until I give notice to Simply Ballet of my desire to withdraw from the program/cancel. Such cancellation will be eeffective at the end of the current month and my enrollment will not automatically renew for the following month. I understand that I can cancel by contacting the Simply Ballet office or canceling in my Membership Online Portal.

 

I understand that my payment information may be requested and stored by Simply Ballet to hold my spot in the free trial class an If I enroll in a class, membership will automatically renew as described above. I understand that my payment information may be requested and stored by Simply Ballet so I can be added to a class waiting list. Registration & payments are powered by Simply Ballet partner software WIX SUBSCRIPTIONS.
